Index corruption on database close

I have 2 different databases where I can reproduce this error.   I have 2 other customer databases where I cannot reproduce this error no matter what I try.  All 4 databases have the same schema.   The error seems to be related to the actual data on the database.

At first, I assumed it was my code using V5RB, but I get this to happen in Valentina Studio so I know it is not my poor coding.
