I reported this to Mantis after first asking about it on this list.   It is reported fixed in version 3.6.  (Or I suppose now, version 4.0.)   A clone of the database fixed the problem for me.  I don't recall trying to add records when this occured, so I can't tell you if that helped in my case.  I no longer have the database.
