[SJHS-Swim] Missing Forms - Urgent Please read!!!

All,

Attached is the file that is available on the SJHS School Fusion page under
Athletics/Swim.

If your name is on the list and you are missing one of the required forms
your child will not be able to swim in Friday's meet.

Registration form is available off the school fusion website, physical's
need to be signed by a doctor and turned in to the Coach Hayden, and the
Emergency card is a blue form that coach passed out two weeks ago. The
emergency card you can get from Mr Hayden or the school, all other forms are
available off the school website.
